Our revolutionary three part diabetes care management system focuses on the three T’s, Tools, Track and Teach.
We deliver the Tools needed for proper glucose monitoring. We enhance this experience with our members by working with them to collect and Track various daily behaviors. We then report these trends and share them with the patient and physician. This collaborative approach provides assurance that members understand how to best care for themselves. We compliment that reporting with education materials to help the patient become more compliant and aware of how to self manage their diabetes.
